What Everyone Should Know About Blood Cancer

Blood cancers don’t always cause obvious symptoms. Knowing the types, possible warning signs, and changes worth discussing with your provider can help you know when it’s time to take a closer look.

When most people hear the word “cancer,” they may picture a tumor or abnormal growth that can be seen on an imaging scan. Blood cancer can be different.

Blood cancers begin in blood-forming tissues, such as bone marrow, or in cells of the immune system. They include a wide range of diseases with different symptoms, rates of progression, treatments, and outcomes. Leukemia, lymphoma, and multiple myeloma are three of the major categories (National Cancer Institute).

Some blood cancers progress rapidly and cause noticeable symptoms. Others can develop more gradually, with early changes that are easy to attribute to stress, aging, a lingering illness, or simply not feeling your best.

That’s one reason awareness matters.

Understanding what blood cancer is doesn’t mean assuming every unexplained symptom is cancer. In fact, many symptoms associated with blood cancers are much more likely to have another explanation. Instead, awareness means knowing what’s normal for you, paying attention when something changes, and having a conversation with your healthcare provider when symptoms are persistent, unexplained, or getting worse.

Detection May Start With Something Routine

Sometimes the first indication that something isn’t quite right comes from a routine blood test.

A complete blood count (CBC) measures different components of the blood, including red blood cells, white blood cells, platelets, hemoglobin, and hematocrit. An abnormal CBC does not automatically mean cancer. Many non-cancerous conditions can cause abnormal blood counts, and not every blood cancer will be discovered through routine bloodwork.

Instead, an abnormal result gives a provider information to consider alongside symptoms, medical history, physical findings, and other test results. Depending on what is found, the next step could be repeating bloodwork, ordering additional laboratory tests, performing imaging, obtaining a biopsy or bone marrow sample, or referring the patient to a specialist.

That’s why routine primary care matters.

A provider who knows your history and has previous results for comparison can look beyond whether an individual number falls inside or outside a reference range. They can consider changes over time and determine whether something deserves further investigation.

What Happens If Something Abnormal Is Found?

Seeing an abnormal result in a patient portal can be frightening, particularly when you don’t know what it means.

But an abnormal blood count is a starting point, not a diagnosis.

Your healthcare provider considers the result within the context of your overall health. They may compare it with previous bloodwork, ask about new symptoms or medications, repeat the test, order additional testing, or determine that a specialist should become involved.

This is also where follow-up becomes especially important.

A test is only useful if someone reviews the result, communicates what it means, and makes sure the appropriate next step occurs. If additional testing or specialty care is recommended, patients shouldn’t be left wondering whom to call or whether anyone is following their progress.

Blood cancer research has highlighted the challenges that can occur along this pathway. A review of diagnostic delays found that blood cancers can be particularly difficult to diagnose because their symptoms are often non-specific and heterogeneous. The researchers emphasized the importance of timely follow-up when patients continue experiencing unexplained symptoms (National Library of Medicine).

Blood Cancer Isn’t Just One Disease

“Blood cancer” is an umbrella term rather than a single diagnosis. There are many different types and subtypes, but three categories account for many of the blood cancers people are most familiar with:

Leukemia

Leukemia begins in blood-forming tissue, such as bone marrow, and results in the production of abnormal blood cells (National Cancer Institute). There are several forms of leukemia, including acute and chronic types, and they can behave very differently. Some progress quickly and require prompt treatment, while others develop slowly and may initially cause few or no symptoms.

Lymphoma

Lymphoma begins in cells of the lymphatic system, which is part of the body’s immune system. The two major categories are Hodgkin lymphoma and non-Hodgkin lymphoma, but there are numerous subtypes within them. Some lymphomas grow slowly, while others are aggressive and require rapid treatment (National Cancer Institute).

Multiple Myeloma

Multiple myeloma is a cancer involving plasma cells, a type of white blood cell found in bone marrow. Abnormal plasma cells can accumulate and interfere with the body’s ability to produce healthy blood cells and perform other important functions (National Cancer Institute).

These differences matter because there is no single experience of blood cancer. Symptoms, risk factors, diagnostic testing, treatment, and prognosis vary considerably depending on the specific disease and the individual patient.

Blood Cancer Can Be Easy to Miss

One of the challenges of recognizing blood cancer is that many of its possible symptoms aren’t unique to cancer.

Fatigue after a busy week may not seem unusual. Neither does getting sick more frequently, noticing an unexplained bruise, waking up sweaty, or experiencing aches and pains. Individually, symptoms like these can have many possible explanations.

Research suggests that this lack of specificity can contribute to delays in diagnosing some blood cancers. A review examining diagnostic delays found that patients sometimes interpreted symptoms as part of their normal health, a mild or chronic illness, or a general feeling of being unwell. Researchers also identified challenges after patients sought care, including non-serious interpretations of symptoms, discontinuity of care, and other barriers along the diagnostic pathway (peer-reviewed research available through the National Library of Medicine).

Another study examining leukemia, lymphoma, and myeloma found that 16% of symptomatic patients waited more than three months before seeking medical attention, with failure to recognize the potential seriousness of symptoms among the factors associated with delays (British Journal of Cancer study available through the National Library of Medicine).

That doesn’t mean common symptoms should automatically be treated as signs of cancer. It means persistent or unexplained changes deserve attention, especially when symptoms don’t improve, become more frequent, or begin occurring together.

Blood Cancer Symptoms People Often Overlook

The symptoms associated with blood cancer vary by type, and some people may have few or no noticeable symptoms early in the disease. However, changes that may warrant a conversation with a healthcare provider can include:

  • Persistent or unexplained fatigue
  • Frequent or recurring infections
  • Unexplained fever
  • Night sweats
  • Unintentional weight loss
  • Easy or unexplained bruising or bleeding
  • Swollen lymph nodes
  • Bone or joint pain
  • Shortness of breath
  • Pale skin
  • Persistent itching
  • Abdominal discomfort or a feeling of fullness

Symptoms alone cannot tell you whether you have blood cancer. Fatigue, for example, can be associated with everything from poor sleep and stress to anemia, infections, thyroid disorders, and many other conditions.

What matters is the pattern. Has the symptom persisted longer than expected? Is it happening more frequently? Is it getting worse? Are several unexplained symptoms occurring at the same time? Those are good reasons to stop explaining a change away and start a conversation with your healthcare provider.

Not All Blood Cancers Look the Same

Even the phrase “blood cancer symptoms” can be misleading because different diseases can present very differently.

For example, leukemia can interfere with normal blood cell production, potentially contributing to symptoms such as fatigue, infections, or bleeding depending on the type and stage of disease. Lymphoma may cause enlarged lymph nodes along with symptoms such as fever, weight loss, or night sweats. Multiple myeloma can affect bones, kidneys, blood counts, and immune function, which means its initial symptoms can look very different from those of lymphoma or leukemia.

And sometimes there are no obvious symptoms at all.

Research examining diagnostic pathways for hematologic cancers found that some people with slower-growing diseases were asymptomatic when diagnosed. In the study, approximately 53% of patients with chronic lymphocytic leukemia were asymptomatic at diagnosis, and some were diagnosed incidentally after blood tests performed during routine health checks or care for another condition (peer-reviewed research available through the National Library of Medicine).

That finding reinforces an important point: feeling fine isn’t always the same thing as knowing what’s happening with your health.

Who Is at Risk?

There isn’t one universal risk profile for blood cancer.

Risk factors differ considerably among leukemia, lymphoma, multiple myeloma, and their many subtypes. Depending on the disease, factors may include increasing age, certain genetic conditions, family history, immune system conditions, previous cancer treatments, or exposure to certain substances.

But risk factors aren’t predictions.

A person can have one or more risk factors and never develop blood cancer. Someone without an obvious risk factor can still receive a diagnosis.

Age is particularly complicated. The National Cancer Institute notes that leukemia occurs most often in adults older than 55, but leukemia is also the most common cancer among children younger than 15 (National Cancer Institute). Other blood cancers have their own age patterns and risk profiles.

Rather than trying to determine whether you’re “at risk enough” to pay attention, focus on something more practical: knowing your health history, keeping up with routine healthcare, and discussing persistent or unexplained changes with your provider.

Don’t Wait Until Symptoms Become Severe

Most fatigue isn’t leukemia. Most swollen lymph nodes aren’t lymphoma. Most back pain isn’t multiple myeloma. But that doesn’t mean persistent symptoms should be ignored. One of the most useful things you can do for your health is develop a sense of what’s normal for you. When something changes and doesn’t return to normal, bring it up.

That might mean saying:

“I’ve been much more tired than usual for the past month, even though I’m sleeping normally.”

“I’m bruising more easily, and I don’t know why.”

“This lymph node has been swollen for several weeks and hasn’t gone away.”

“I’ve been having night sweats repeatedly, and that’s new for me.”

The goal isn’t to diagnose yourself. It’s to give your healthcare provider the information they need to decide whether further evaluation is appropriate. And if symptoms continue despite an initial explanation, keep the conversation going. Persistent symptoms deserve persistent attention.
